We take Visa, Mastercard, American Express, Discover, debit cards, checks, and cash.
 

Scheduled appointments can be cancelled up to one day prior, before 3 PM. Otherwise, a no-show fee of $35 will be charged.

We do offer payment plans. If you would like more information about our options, please call our office and ask for our billing department.

If you are paying cash or have a co-pay associated with your insurance plan, that is due at check-in.

Yes. Please bring your insurance card (if you are not self-pay) and your photo ID with you. Also bring a list of any medications you are taking, which includes the dosage and how often you take them. Any office notes or operative reports that are related to this condition will also be helpful at your visit. If you have had any imaging done (for this condition only) outside of our office within the last 3 months (CT, MRI, XRAY, etc) please obtain those images on CD from the imaging center and bring that CD with you to your appointment. If you have had recent imaging and it is not with you when you come, you may be required to have new imaging done during your visit.

Our preferred method for obtaining patient information is via our electronic Patient Portal. Through this system you can enter most of the information we need and this eliminates time-consuming written paperwork. If you don’t have access to a computer you can come early and enter the data through our Patient Kiosk in our office. If you have questions, a member of our staff can assist you when you arrive.
